Use a Cause & Effect diagram to show Why events occurred.

One Cause per box and be complete in terms of Time, Location & Magnitude.

AND & OR are the only two logical operators allowed.

Take small steps down the diagram so that anyone can follow the logic.

Sometimes, in order to know Why, we have to understand How:

• Example:

  • To have a fire, you need a flammable material, ignition source and oxygen (in the correct proportions).

  • For a leak, you need a path, a product and a pressure difference.

Once developed, share and test the logic with relevant stakeholders
