Despite best efforts at identifying root causes, we often find that incidents repeat themselves. This comes at considerable cost to the business, environment or sometimes to the lives of people.
We may also experience continued undesirable outcomes after making various interventions. These erode efficiency, safety and ultimately profits.
By using a technique that is designed to establish underlying actual and potential causes, we are able to better mitigate against future incidents and undesirable performance.
The course is more than just an incident analysis tool. By applying the cause & effect analysis techniques, individuals are able to better support the day-to-day functioning of the business. This is accomplished by better decision-making and problem resolution outside of formal investigations.
The course can be tailored depending on client needs. We offer courses spanning from half a day (basic troubleshooting), 2 days (basic facilitation) to 1 week for fully trained incident facilitators.

The course is designed for any individual that would be required to facilitate an investigation into any area of the business. This could include staff from Human Resources, Engineers, Managers, Supervisors, Artisans, Technologists.
We also offer shorter introductory courses that are suitable for people that may be involved in investigations or decision-making, but are not required to facilitate formal investigations.
The course is highly interactive with learners being encouraged to bring their own experiences into the discussions.
We also try to use actual incidents experienced by the client to ensure that the material is relatable to the class.
The material is presented in a variety of formats, from videos and presentations to case studies and discussion groups.
We prefer face-to-face classroom formats, but the course can also be presented online if required.
We also offer continued mentorship and support as learners hone their skills outside of the classroom. This may involve advice, investigation support, report reviews, etc.
