Beware of barrier or solution type of analysis. This is evident by the use of words like “not”; “wasn’t”; “failed”; “didn’t”; “should”; “insufficient”; etc.
Look for what happened or what was there as opposed to what was supposed to happen, what failed, or what was expected to be present.
Be alert to reasoning using hindsight. This is a type of analysis that is done after the fact and does not add value to trying to understand the cause at the time of the event.
Also avoid an emotional response to an observation. This may lead to an incorrect assumption. Rather try to articulate the observation that led to that thought.
An observation is something made by one of your 5 senses (sight, hearing, touch, smell or taste).
