  • Location

  • Consequences (quantify if possible)

  • Any information about concurrent activities

• In-Scope & Out-of-Scope clarification

• Expected milestone dates (e.g. initial findings; Cause & Effect diagram; 1st Stakeholder engagement; Solution proposal; Investigation close; Sharing of Learnings). Note that these may change during the course of the investigation.

• Team composition: Team Leader; Facilitator; Members

• Expectation on feedback from Team Leader with regards to progress
